Revision breast surgery is performed using general anesthesia. In some breast revision procedures, as well as some primary mastopexy procedures, Dr. Walters may recommend the use of the Galaflex Scaffold or DuraSorb bioabsorbable surgical mesh to help support breast tissue that has lost elasticity. Often referred to as an “internal bra.” Galaflex or DuraSorb help to stabilize and strengthen the soft tissue by creating tissue regeneration in a targeted manner. These unique 3D monofilament scaffolds are made from biologically derived poly-4-hydroxybutyrate (P4HB), which naturally degrades and is non-toxic, well-tolerated by the body. The addition of the supportive scaffold helps to ensure against “bottoming out” of the implant in patients with thinner breast tissue.

Breast revision surgery removes an implant that is either not functioning properly or no longer desired. Breast revision surgery may also be performed to change implant size or go from an implant to a fat transfer (or vice versa). There are other common situations where a revision surgical treatment is desired or required:
Even though this is rarely seen, breast implant rupture can occur. When a saline implant type ruptures, the implant will deflate almost immediately. As a result, a visible deflation will be noticeable. Meanwhile, if the implant is silicone-based and ruptures, it is only seen via an MRI. Whether silicone or saline, when a rupture occurs, a secondary procedure will be required.
This is known as the hardening of scar tissue, occurring when the delicate tissue surrounding the saline or silicone implant becomes thick and hard. Although many implant surgeries do create some internal scar tissue, most women will never see or feel it. In some situations, the hardening tissue can interfere with implant placement, altering the breast shape and implant functionality, resulting in an undesirable outcome. Capsular contracture surgery to clear away scarring would be recommended.
This condition occurs when the implant sizes look disproportionate. This may be due to an incorrect size implant, insufficient tissue on one side, or a different condition that causes asymmetry. An assessment and diagnosis would need to take place to determine what is leading to the asymmetry; then, revision surgery would be necessary to resolve the problem, and, finally, a new set of breast implants would be positioned to create a symmetrical result.
This problem happens if one or both implants bottom out or drop too low. Sometimes this is a case of a positioning error from the original surgical procedure. To modify this, a different placement method may be needed. For many patients, going to a silicone breast implant will solve this issue. Additionally, the implant pocket may be tightened, known as capsulorrhaphy, and an internal bra like Galaflex or DuraSorb may be used to support the new implant.
Symmastia is a condition that takes place when breast implants are inserted and placed too closely together. Most often, this is because the breast implant had the incorrect shape and width. A revision treatment will repair this problem by utilizing an implant that fits your frame and is aesthetically pleasing. The placement of a bioresorbable scaffold, or “internal bra,” may also be recommended.
This condition is characterized by the presence of obvious rippling and wrinkles beneath the skin. This irregularity typically forms just along the edging of the implant. Rippling is generally observed in patients with low body weight. Simply switching to a silicone breast implant can often fix this issue. Additionally, fat grafting may be recommended to create a smooth transition between the breast implant and the natural breast tissue.
Sometimes, a secondary breast surgery is as straightforward as getting a smaller or larger-sized breast implant. During the first surgery, you may have opted to go too conservative or too large. A different implant size can be switched with the original to reach the desired aesthetic goal.
